Terms of use

The service

elev8care is a clinician-level balanced scorecard for primary-care organizations, operated by London Health Partners, London, Ontario. By signing in or using the service you agree to these terms. If you use elev8care on behalf of an organization, its agreement with us also applies.

Accounts are by invitation

Accounts are created only by invitation from an organization that uses elev8care. Keep access to your email account secure — sign-in links are sent there — and tell us at once if you think someone else has used your account. Your organization can change or remove your access at any time.

No identifiable patient information

Do not upload, type or attach identifiable patient information — including names, health card numbers, chart numbers, dates of birth and addresses. Use pseudonymous identifiers only. Your organization is responsible for de-identifying data before it reaches elev8care.

Acceptable use

• Use the service only for your organization's quality and performance work.
• Do not try to access another organization's data or bypass access controls.
• Do not disrupt the service, probe it for vulnerabilities without our written permission, or use automated tools to extract data from it.
• Do not upload content you have no right to share.

AI output is decision support

Recommendations and chat answers are generated by AI from your organization's data. They support quality-improvement decisions and are not clinical advice. They can be incomplete or wrong; review them against the underlying scorecards and your own professional judgment before acting.

Your data

Your organization keeps ownership of the data it uploads. We use it only to provide the service, as described in the privacy policy.

Service provided as is

We work to keep elev8care available and accurate, but the service is provided as is, without warranties of any kind, to the extent the law allows. We are not liable for indirect or consequential losses arising from its use. We may change or suspend features, and will post changes to these terms on this page with a new revision date.

Governing law

These terms are governed by the laws of the Province of Ontario and the federal laws of Canada that apply there.
